You are invited to attend the 2016 Patient Safety Forum jointly provided by the Massachusetts Coalition for the Prevention of Medical Errors and Healthcentric Advisors.

The Forum will recognize and share effective work being done in Massachusetts to promote a culture of safety and prevent medical errors. The format will include several panels with “rapid-fire” presentations from hospital, ambulatory and other settings.

7:30 AM: Registration and a continental breakfast

8:00 AM-12:30 PM: Program

This activity has been planned and implemented in accordance with the accreditationrequirements and policies of the Accreditation Council for Continuing Medical Education through joint providership of Healthcentric Advisors and the Massachusetts Coalition for Prevention of Medical Errors. Healthcentric Advisors is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
